American Traveler is looking for a Physical Therapist for a Home Health assignment requiring 1+ year of recent experience and proficiency with OASIS and HCHB.

Details

• Home health setting serving a geriatric patient population
• Caseload averages 5-7 patients per day, with 6 visits daily or a minimum of 30 points weekly
• Day shift schedule, 4x10-hour shifts from 8:00 AM to 6:00 PM
• Territory covers up to a 60-mile radius from the hub office
• No on-call requirement, though rotation may be discussed during interview
• iPad provided for documentation; mileage reimbursed at IRS standard rate

Requirements

• Minimum 1 year of recent Physical Therapy experience required
• BLS certification required
• OASIS experience required for completing Start of Care (SOC) visits
• Proficiency in HCHB (Homecare Homebase) EMR required
• Active driver's license and personal transportation required
• License required if applicable
• Skills across orthopedic and neurologic patient populations, including post-op recovery, spinal conditions, stroke rehabilitation, and traumatic brain injury
• Experience with home exercise programs and patient/family education

Additional Information

• Responsibilities include timely communication and documentation of patient care
• Monday orientation includes completion of required, billable training modules
• Hospice/Home Health checklist completion is mandatory
• Dress code requires scrubs of any color; jeans are not permitted
• No first-time travelers considered for this position
• RTO requests must be submitted at the time of application
• Travel pairs accepted, provided each traveler is assigned their own territory and has personal transportation
• Former employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Local candidates accepted at the travel rate
