Description

American Traveler is looking for a Physical Therapist to provide Home Health services for geriatric patients, requiring OASIS experience and 1+ years of recent PT experience.

Details

• Home health setting serving a geriatric patient population
• Day shift schedule, Monday through Friday, no weekends
• 5x8-hour shifts, 8:00am to 5:30pm
• Average caseload of 5-7 patients per day
• Guaranteed 30 visits weekly for a 40-hour weekly guarantee
• All charting must be completed by 9am the following morning
• EMR: HCHB preferred
• Coverage area spans up to a 60-mile radius across parts of Walla Walla, Columbia, and Umatilla counties
• On-call rotation may be required

Requirements

• Current BLS certification required
• Minimum 1 year of recent, relevant PT experience required
• OASIS experience required for completing Start of Care assessments
• Proficiency in HCHB (Homecare Homebase) required
• Active driver's license and personal transportation required
• License required if applicable, with license in-hand required for both WA and OR at time of consideration
• Experience with orthopedic and neurologic patient populations, including post-op and post-stroke rehabilitation

Additional Information

• Responsibilities include home exercise program development and patient/family education
• Timely documentation and communication with the care team are mandatory
• Hospice/Home Health checklist completion is required
• Monday orientation with billable modules completed during this time
• iPad provided; personal cell phone use not permitted
• Mileage reimbursed at the IRS standard rate
• Scrubs required in any color; no jeans permitted
• First-time travelers will not be considered for this role
• Travel pairs accepted, though each traveler is assigned their own territory and must have independent transportation
• Local candidates accepted at the travel rate
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Requested time off (RTO) must be submitted at the time of application
