American Traveler is seeking a Home Health Physical Therapist with 1+ year of recent experience and required OASIS proficiency for a geriatric caseload.

Details

• Home Health setting serving a geriatric patient population
• Caseload averages 5-7 patients per day, with a 30-visit weekly guarantee for a 40-hour week
• Monday through Friday schedule, no weekends
• Day shift, 8:00 AM to 5:30 PM
• May require participation in an on-call rotation
• Coverage area spans up to a 60-mile radius across parts of Walla Walla, Columbia, and Umatilla counties
• All charting must be completed by 9:00 AM the following morning

Requirements

• Minimum 1 year of recent Physical Therapy experience required
• BLS certification required
• OASIS experience required for completing Start of Care assessments
• Proficiency in HCHB (Homecare Homebase) EMR preferred
• Active driver's license and personal transportation required
• License required if applicable
• Timely documentation and communication skills required

Additional Information

• Responsibilities include home exercise programs and patient/family education
• Treats conditions including orthopedic, neurologic, and post-surgical cases
• Monday orientation includes completion of billable modules
• Hospice/Home Health checklist completion is mandatory
• Scrubs required in any color; jeans not permitted
• No cell phone provided, but an iPad is issued
• Mileage reimbursed at the IRS standard rate
• First-time travelers will not be considered for this role
• Travel pairs accepted, though each traveler must have their own territory and transportation
• Former employees must be separated from the facility for at least 6 months and approved by HR before returning as a traveler
• Local candidates accepted at the travel rate
• Requested time off must be submitted at the time of application
