Description

American Traveler is seeking a Physical Therapist for a home health assignment requiring at least 1 year of home healthcare PT experience and licensure in VT and NH.

Details

• Home health setting covering more than 70 towns across VT and NH, including White River Junction VT, Lebanon NH, Bradford VT, and Windsor VT areas
• Full time, day shift schedule
• Average patient visit length is 45 minutes
• Work as part of an interdisciplinary care team while maintaining independence in patient visits

Requirements

• BS in Physical Therapy required
• Current BLS certification required
• Minimum 1 year of experience as a PT in a home healthcare setting
• Must hold a PT license in VT and NH, or an unencumbered compact license - license required if applicable
• ASHA, CHT, or ACP training certifications preferred

• Responsibilities include screening, evaluating, re-evaluating, and treating patients, developing and carrying out treatment plans, documenting evaluations and goals, and teaching patients and families about care rationale
