Description

American Traveler is seeking a Physical Therapist Assistant for a home health assignment requiring 1 year of home health experience and Homecare Homebase EMR proficiency.

Details

• Home health setting covering Northern Charlotte, Huntersville, Cornelius, Davidson, Morrisville, and Salisbury
• Caseload of approximately 6 patient visits per day with 60-80 miles of driving daily, mileage reimbursed weekly
• Patient population includes chronic diseases, orthopedic/ortho surgery including total hip and knee replacements, falls, weakness, CHF, COPD, and occasional stroke patients
• Day shift schedule, Monday through Friday, 8:30am-5pm
• No weekend rotation and no on-call requirements
• Uses Homecare Homebase (HCHB) as the required EMR, Epic Rover for view-only access, and Focura for uploading discharge instructions and wound documentation

Requirements

• Active PTA license in North Carolina required if applicable
• Current BLS certification required
• Minimum 1 year of home health experience required
• Valid driver's license and proof of insurance required
• Homecare Homebase (HCHB) EMR experience required with no exceptions
• Skills in gait training, balance training, and non-pharmaceutical pain management required
• Last 4 digits of SSN and full date of birth required for consideration
• At least one professional reference dated within the past year from a Supervisor, Manager, or Director

• Facilitate patient plans of care to achieve highest level of functioning in the most efficient way possible
• Collaborate closely with PT and clinical team members, recognizing signs and symptoms to help prevent hospital readmissions
• Support team includes departmental leadership, scheduler, clinical manager, clinical supervisor, and clinical peers
• Orientation includes New Employee Orientation (NEO) plus 5 days of unit-specific orientation
• Grey scrub color required
• Team-oriented department culture
• No major holiday coverage required
• Time off requests subject to manager approval
• First-time travelers will be considered if seasoned in home health
• Candidates must reside more than 50 miles from the facility to be eligible
• Returning employees must be separated from any AHSE facility for at least 1 year after working a permanent position
