American Traveler is seeking a Home Care Physical Therapist with home health and OASIS experience to provide one-on-one therapy in patients' homes.

Details

• Home health setting providing one-on-one physical therapy care in patients' homes
• Schedule is Tuesday through Saturday, 8am-5pm
• 13-week assignment with 2 open positions
• Guaranteed 36 hours per week
• McKesson/Netsmart EMR used

Requirements

• Active Vermont PT license required (license required if applicable)
• Minimum 1 year of experience required
• Home health experience required
• OASIS experience required, as candidate will case manage
• BLS certification required
• McKesson/Netsmart EMR experience strongly preferred
• Proficiency in standard therapy measurement techniques including ROM, strength, and balance testing required

Additional Information

• Responsibilities include assessing patient conditions and developing individualized care plans to maximize mobility and independence
• Collaborate with an interdisciplinary team to ensure patient needs are met
• Utilize a variety of therapeutic modalities to optimize patient care
• Candidates should have no requested time off during the assignment
