American Traveler is seeking a Physical Therapist Assistant with a Hawaii license and 2+ years of SNF/ICF experience for a long-term care rehab position at a state veterans home.

Details

• Work setting is a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) within a state veterans home
• 13-week contract with a day shift schedule
• Generally scheduled 5 days per week with likely inclusion of one weekend day
• Departmental coverage spans 7 days per week
• Floating to other units or facilities may be required based on census and staffing needs
• EMR systems used include Net Health Rehab Optima and Epic

Requirements

• Current active Physical Therapist Assistant license in the State of Hawaii required; license required if applicable
• Current BLS/BCLS certification required
• Minimum 2 years of experience in a SNF/ICF setting preferred
• Must be available to work Hawaii State and Federal Holidays as required by facility operations

Additional Information

• Provide rehabilitation services including evaluations, reassessments, treatments, and other clinical services consistent with licensure and scope of practice
• Develop and implement individualized treatment plans in collaboration with the supervising therapist, physician, and interdisciplinary team
• Educate residents, families, and caregivers on treatment plans and patient progress
• Complete accurate, timely, and compliant documentation in accordance with facility, state, federal, and payer requirements
• Orientation to the rehabilitation department, EMR systems, and facility workflows is provided
• Candidates not yet holding an active Hawaii license must apply for one upon acceptance of the offer and must have it in hand by the assignment start date
• Candidates must plan to arrive the Thursday prior to the assignment start date to attend an employee health appointment on Friday
