American Traveler is seeking a Home Health Physical Therapist for a float position requiring MA licensure and prior certified home care experience.

Details

• Home health setting, providing PT services in patients' places of residence across the North Region (Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, West End)
• Patient population ranges from birth to over 100 years old with physical impairments, functional limitations, or disabilities
• Schedule is 08:00-16:30 with weekend rotations once every 6 weeks
• Requires ability to travel to assigned cases across the service area
• Uses Epic for charting

Requirements

• Active Physical Therapist license in Massachusetts required (license required if applicable)
• Minimum 2 years of PT experience required
• Minimum 1 year of Certified Home Health Care experience required (hospice, private care, or standalone case management experience does not qualify)
• Minimum 1 year of Case Management experience required
• MA Conditional Licenses not accepted
• Flu vaccine required with no religious or medical exemptions allowed

Additional Information

• Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate care
• Responsible for thorough and timely electronic documentation and interdisciplinary communication per agency standards
• Start date is contingent on credentialing clearance; delays may result in a rescheduled start date
• Local candidates accepted with a permanent residence within 60 miles of Boston
