American Traveler is seeking a Physical Therapist for a 13-week home health travel contract in the New River Valley, VA, managing a full plan of care for adult and geriatric patients.

Details

• Home health setting serving adult and geriatric patients in their homes across the New River Valley, including Christiansburg, Radford, Pulaski, Floyd, and surrounding Montgomery and Giles County communities
• Based out of Blacksburg, VA with field-based visits throughout the service area
• Approximately 25–30 units per week
• Responsibilities span the full episode of care: Start of Care evaluations, treatment planning, progress notes, and discharge
• Documentation completed in the field using WellSky on a company-issued tablet
• Independent, community-based work environment with no fixed clinical site

Requirements

• Active Virginia PT license required (license required if applicable)
• Minimum 1 year of clinical PT experience
• Home health and OASIS experience preferred but not required
• Valid driver's license and reliable personal transportation required
• Ability to work independently in patients' homes
• Comfort with field-based electronic documentation

Additional Information

• Mileage reimbursement provided for travel throughout the service area
• Company-issued tablet provided for field documentation
• Option to extend or convert to a permanent position at the end of the contract
• All time off and scheduling requests must be submitted at the time of initial consideration
• 2 professional references from a supervisor within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration
