American Traveler is seeking a Home Health Physical Therapist with at least 1 year of experience to provide services across a 3-county territory in South Carolina, working with an adult to geriatric patient population.

Details

• Home health setting serving patients across Barnwell, Aiken, and Edgefield counties in South Carolina, with the majority of patients located in Aiken and Barnwell counties
• Patient population ranges from adults to geriatrics, with common diagnoses including joint replacements, COPD, CHF, and T2DM
• Approximately 6 visits per day with an average daily census of 175; full-time productivity is around 25 units
• Mileage is reimbursed at the IRS standard rate and tracked through HCHB; anticipated daily driving is generally 20–30 miles, with an estimated maximum of around 70 miles on higher-volume days
• Day shift, 5x8-hour schedule (8:00–16:30); no weekend shifts
• EMR: Home Care Home Base (HCHB) highly preferred; facility is transitioning to Epic around September 2026
• Floating within scope of practice may be required as needed
• 12-week contract

Requirements

• Minimum 1 year of home health PT experience required
• Active SC PT license or Compact PT license (compact members should verify state eligibility) required; license must be active at time of consideration
• Current BLS certification required
• HCHB experience highly preferred; candidates without HCHB experience must complete an online HCHB course prior to starting (course fee is the traveler's responsibility)
• Copies of required certifications must be provided for consideration

Additional Information

• Scheduler groups patient visits geographically by day to minimize drive time and support productivity
• Any color scrubs are acceptable
• RTO maximum of 7 days for the assignment; RTO cannot be requested during the first week of the assignment
• Holiday coverage is required: if 1 or 2 major holidays fall within the assignment period, the traveler is expected to work all of them; if 3 major holidays fall within the assignment period, the traveler is expected to work 2 of them — major holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Year's Eve/New Year's Day
• Travelers must be away from all facilities in this health system for a full year before returning as a traveler
• Local candidates are accepted at the same rate
• First-time travelers are welcome to apply
• All candidates undergo a clinical screening review prior to manager consideration
