Description

American Traveler is hiring a compassionate Hospice RN to provide skilled end-of-life nursing care to patients in their homes, with a minimum of 3 years of experience and an active UT or compact RN license required.

Details

• Home-based hospice care setting, with care delivered in patients' preferred place of residence
• Serves terminally ill patients requiring pain and symptom management, end-of-life support, and individualized care planning
• Day shift schedule
• On-call rotation required, approximately 1–3 shifts per week including 3 weekends over the 13-week contract
• Must be able to respond within 45 minutes to patient needs when on call
• Valid driver's license and reliable transportation required for home visits
• Hospicesoft/Careficient EMR experience preferred
• Holiday coverage required (New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Day)

Requirements

• Active RN license in Utah or a compact state currently in hand
• Minimum 3 years of nursing experience, preferably in hospice, home health, or palliative care
• Current BLS certification required
• Strong clinical assessment skills
• Valid driver's license and reliable personal transportation

Additional Information

• Perform comprehensive patient assessments and coordinate individualized hospice care plans
• Provide pain and symptom management in accordance with each patient's plan of care
• Educate and support patients and families on the disease process, self-care techniques, and end-of-life issues
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and volunteers
• Maintain accurate and timely documentation in compliance with state, federal, and organizational policies
• Blue scrubs preferred
• Pre-employment drug test and background check required
