Description

American Traveler is seeking a Physical Therapist with a minimum of 3 years of experience for a 13-week contract at a long-term care/skilled nursing facility.

Details

• Long-term care and skilled nursing facility setting focused on rehabilitation therapy
• Day shift schedule
• Responsibilities include evaluation and treatment of residents to maximize functional abilities and independence in activities of daily living
• Collaborative, interdisciplinary team environment including physicians, nursing staff, and other therapy professionals
• Documentation performed in an Electronic Medical Record (EMR) system

Requirements

• Minimum 3 years of physical therapy experience required
• Bachelor's degree required; Master's or Doctorate preferred
• Active PT license in the applicable state required; license required if applicable
• BLS/BCLS certification required
• Prior experience in a long-term care or skilled nursing facility preferred
• Strong verbal, written, and documentation skills required
• Ability to lift up to 50 lbs; lifts above 50 lbs require a two-person assist
• Must be able to perform a combination of sitting, standing, walking, bending, stooping, twisting, and reaching
• Proficiency in Microsoft Office required

Additional Information

• Responsibilities include developing individualized plans of care with short- and long-term functional goals, frequency, and duration of services
• Provides consultation to referring physicians and attends pertinent team meetings regarding resident care and discharge planning
• Recommends and trains residents on appropriate assistive devices and provides education to residents and caregivers
• Supervises PT Assistant-level staff in accordance with state mandates and professional standards
• Provides mentorship to new staff during orientation and assists with department competency completion
• Supports compliance with Medicare, Medicaid, Private Pay, and other reimbursement documentation requirements
• Actively participates in the development of new rehabilitation programs and services
• Maintains department cleanliness and adheres to infection control procedures
