American Traveler is seeking an experienced RN Hospice Liaison for a home health/hospital liaison role in the Milwaukee, WI area, requiring Epic proficiency and a minimum of 2 years of hospice experience.

Details

• Home Health Liaison role based out of an office location in West Allis, WI, with daily travel to assigned Aurora hospital(s)
• Serves as a liaison for hospice referrals from hospitals and SNFs, with an average caseload of 2-3 patients per day
• Adult patient population with diagnoses including heart failure, cancer, and other end-of-life disease processes
• May be assigned to 1-2 Aurora hospitals per day depending on hospital size and census; average driving time is approximately 45 minutes one way
• 10-hour day shift, Monday–Friday, 0800–1800, with one rotating day off per week
• No weekend rotation, no on-call requirements, and no holiday coverage expectations
• EMR: Epic (Hyperspace and Epic Remote Client) — experience required

Requirements

• Active WI RN license or compact RN license currently in hand
• Current BLS certification required
• Minimum 2 years of hospice experience required; hospice liaison experience preferred
• Epic EMR experience required
• Strong skills in end-of-life symptom management including pain and dyspnea, and comfort with end-of-life medications
• Ability to educate providers and collaborate effectively with physicians regarding hospice benefits, philosophy, and levels of care
• Valid driver's license, current auto insurance, and personal vehicle required
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN collected within the past year
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible

Additional Information

• Responsibilities include monitoring the referral queue, conducting patient assessments, and providing recommendations on the appropriate hospice setting for patients transitioning from hospital or SNF environments
• Liaisons may admit patients to hospice services directly within the hospital setting
• Laptop and phone provided
• Support available from supervisor/manager via phone and chat, the liaison team, and Hospice Medical Directors
• Orientation includes facility NEO and 40 hours of unit orientation with a staff member
• Business casual attire or professional scrubs required; white coat preferred but not required
