American Traveler is seeking a Home Health Physical Therapist with OASIS, home health, and hospice experience for a 13-week travel contract in Vermont.

Details

• Home health and hospice setting with patient-to-patient home visits
• Monday through Friday, 8:00 AM – 5:00 PM schedule
• Holiday coverage required as needed
• Uses HealthWyse EMR

Requirements

• Minimum 2 years of physical therapy experience required
• OASIS experience required
• Home health and hospice experience required
• Must have experience traveling between patient homes for in-home visits
• Active Vermont PT license required if applicable
• Must be able to manage and adapt to schedule changes

Additional Information

• Responsibilities include evaluation and treatment of patients in a home health and hospice environment
• Must be flexible and self-directed given the nature of home visit scheduling
