American Traveler is seeking an experienced RN for a Weekend Field Nurse position in a PACE (Program of All-Inclusive Care for the Elderly) home health setting, requiring at least one year of experience with frail or elderly populations and weekend availability.

Details

• Weekend-only schedule: Saturday and Sunday, 7:00am–11:00pm
• Home health and field-based setting within a PACE program serving geriatric participants
• Caseload includes routine, acute, and non-acute visits to participant homes and care facilities
• Procedures include wound care, IV hydration, lab draws, full-body assessments, and other nursing tasks
• Requires use of EMR for documentation during field visits
• Coordination with PCPs, DME suppliers, ALFs, SNFs, pharmacies, paramedics, hospitals, and other care partners
• 13-week contract

Requirements

• Active Michigan (MI) RN license required
• Current CPR certification (AHA) required, or willingness to obtain
• Associate's degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred
• Minimum 1 year of experience working with frail, elderly, or long-term care populations required
• Home care experience preferred (minimum 1 year)
• Wound care experience preferred (minimum 1 year)
• Must possess a valid Michigan driver's license, reliable transportation, and current car insurance
• Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B
• Must be 18 years of age or older
• DOB and SSN required for consideration

Additional Information

• Conduct in-person participant assessments to determine appropriate level of care (emergent, urgent, non-urgent, or home care status)
• Respond promptly to calls from PCPs, triage, and support staff; document all contacts and follow-ups
• Provide education and support to participants, family members, DPOAs, and facility staff
• Initiate and document emergency treatment, new orders, incident reports, fall documentation, and other quality measure policies
• Participate in new employee orientation, in-service programs, staff meetings, and continuous quality improvement initiatives
• Must adapt to changing workloads and maintain open communication with leadership
• Holiday coverage may be required as part of weekend schedule demands
