American Traveler is seeking an experienced RN Case Manager for a home health position in the Milwaukee North area, requiring at least 1 year of home health experience and a WI or compact RN license.

Details

• Field-based home health position serving patients in community settings including private homes, family member homes, assisted living facilities, group homes, and nursing homes in Ozaukee and Washington counties, WI
• High-acuity patient population including those with pleurx drains, PCA pumps, IV antibiotics, TPN, Foleys, wounds, wound VACs, chest tubes, and various drains
• RN Case Manager caseload of 22–25 patients per week with approximately 6 visits per day
• 8-hour day shift, Monday–Friday, 8:00 AM–4:30 PM; must be available by phone from 8:00 AM–5:00 PM
• Daily team huddle at 7:50 AM, which can be attended remotely by phone or Microsoft Teams
• Flexible scheduling with M–F or Tue–Sat options; candidates open to occasional Saturday work are preferred
• No weekend rotation, no on-call, and no holiday coverage required
• Epic EMR used; experience preferred but not required
• Floating requirements may vary

Requirements

• Active WI RN license or compact RN license currently in hand
• Current BLS certification required
• Minimum 1 year of home health RN experience required, with most recent experience within the last year
• Knowledge of and ability to troubleshoot pleurx drains, nephrostomy tubes, trachs, PCA pumps, and gastric suction equipment
• Skills in patient and family education, provider communication, and disease management
• Last 4 digits of SSN and full date of birth required for consideration
• Minimum 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN

Additional Information

• Responsibilities include managing a home health patient caseload, conducting field visits, coordinating care with providers, and supporting patient education and disease management
• Support team includes fellow case managers, providers, a clinical manager, chaplain, and social workers
• On-site training provided for equipment troubleshooting if needed
• Orientation is up to 2 weeks based on individual comfort level
• Laptop, cell phone, and PPE are provided
• Candidate must be N95 fit tested prior to start
• No specific scrub color requirement
• First-time travelers are welcome to apply with at least 1 year of home health experience
