Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent experience, OASIS and HCHB proficiency, and flexibility to cover a wide service territory in Alpena, MI.

Details

• Home health setting serving a geriatric patient population
• Caseload of approximately 5-7 patients per day (to be confirmed during interview)
• Service territory covers up to a 60-mile radius from the hub/office
• Day shift schedule, 5x8-hour shifts (8:00 AM - 5:00 PM)
• On-call required every 3rd to 4th weekend
• EMR: HCHB (experience required)
• iPad provided for documentation in the field

Requirements

• Minimum 1 year of recent home health PT experience required
• OASIS experience required
• HCHB EMR experience required
• Current BLS certification required
• Active PT license required if applicable
• Must be flexible with service territory coverage

• Mileage is reimbursed at the IRS standard rate
