Description

American Traveler is seeking a licensed Physical Therapist with a minimum of 1 year of home health experience for a 13-week home health assignment serving adult patients across multiple communities in North Carolina.

Details

• Home health setting serving adult patients in and around Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding areas
• Patient population includes adults with chronic diseases, orthopedic conditions (including total hip/knee replacements), falls, weakness, CHF, COPD, and stroke
• Expected caseload is approximately 6 patient visits per day, with an average driving distance of 60–80 miles per day
• Therapists schedule patient visits the day prior to the planned visit
• Required weekly visit equivalent (VE) target is 30; cancelled appointments do not count toward VEs
• Day shift, 12-hour shifts (0800–2000), 36 hours per week
• No weekend rotation, no on-call requirements, and no major holiday coverage required
• EMR systems include Home Care Home Base (required), Epic Rover (view only), and Focura for uploading discharge instructions and clinical documents
• A company cell phone (Samsung Note 9) is provided for documentation and patient/provider communication

Requirements

• Minimum 1 year of home health experience required, with most recent experience within the last year
• Experience with geriatric and/or pediatric populations and chronic disease management required
• OASIS-E experience required
• Home Care Home Base EMR experience required
• Proficiency with Epic preferred
• Current BLS certification required
• Active state PT license required; license required if applicable
• Valid driver's license and proof of auto insurance required
• Gait training, balance training, and non-pharmaceutical pain management skills required
• Ability to create and manage plans of care to advance patients to their highest functional level
• Strong communication skills with clinical teams and ability to recognize signs and symptoms to prevent hospital readmissions
• Last 4 digits of SSN and full date of birth required for consideration
• Minimum 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN

Additional Information

• Candidates must reside at least 50 miles from the facility; local candidates are not accepted
• First-time travelers will be considered if they have solid home health experience
• Former employees of any AHSE facility must have been separated from a permanent position for at least 1 year before being eligible
• RTO is subject to manager approval
• Support structure includes departmental leadership, a scheduler, clinical manager, clinical supervisor, and clinical peers
• 5-day on-site orientation provided
