American Traveler is seeking a Physical Therapist for a Home Health position covering the Sacramento, Modesto, and Stockton areas, requiring a minimum of 1 year of home health experience and a valid state license.

Details

• Home health setting serving geriatric patients, with cases including wound care and general home health
• Coverage area spans the Sacramento, Modesto, and Stockton regions, with potential travel to surrounding counties as discussed during interview
• Minimum of 23–24 patient visits per week
• Day shift schedule, Monday–Friday, 8:00 AM – 5:00 PM (5x8 hours)
• No weekend shifts required
• On-call is not required; discretionary only
• Charting is performed using Kinnser/WellSky; all charting must be completed on-site using a facility-provided tablet
• Contract length is 4–6 months; less experienced clinicians are required to complete a 6-month contract (non-negotiable)
• Mileage is reimbursed at the IRS standard rate from first to last patient visit (excludes travel to/from home)

Requirements

• Minimum 1 year of home health experience and 2 years of overall clinical experience as a PT required
• Valid state PT license required; license required if applicable
• Current BLS certification required
• No pending licenses accepted; valid license must be in hand at time of consideration
• Self-motivated with strong time management and punctuality
• Ability to complete charting efficiently and on-site

• Compliance onboarding may take 2–3 weeks; all required documents must be cleared before patient care can begin
