Description

American Traveler is seeking a compassionate and skilled Physical Therapist (PT) for a 13-week assignment in a Skilled Nursing Facility (SNF) setting, with experience in geriatric rehab preferred.

Details

• Skilled Nursing Facility (SNF) setting, encompassing inpatient, long-term care, and short-term rehabilitation
• Primary patient population includes geriatric residents with mobility limitations, functional deficits, and varying levels of independence
• Day shift schedule, 9AM–5PM

Requirements

• Graduate of an accredited Physical Therapy program
• Current, active state PT license required; license required if applicable
• CPR certification required or preferred depending on facility policy
• Experience in a SNF or geriatric rehab setting preferred
• Strong communication, documentation, and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Must be able to perform physical tasks including lifting, standing, walking, and assisting patients with transfers and mobility

Additional Information

• Responsibilities include conducting thorough patient assessments, developing and implementing individualized treatment plans, and delivering evidence-based PT interventions targeting strength, coordination, balance, and mobility
• Collaborates closely with physicians, nursing staff, occupational and speech therapists, and family members to coordinate comprehensive patient care
• Required to monitor and document patient progress in compliance with state, federal, Medicare/Medicaid, and facility guidelines
• Participates in care planning meetings and interdisciplinary team conferences
• May supervise PTAs and therapy aides as needed
• Patient and caregiver education on safe mobility techniques, home exercises, and adaptive equipment is a key component of the role
