• Day shift (AM), 8-hour shifts
• 13-week contract

Requirements

• Active Physical Therapist license required; compact license accepted at start, but full Indiana state license must be obtained during the assignment
• SSN required for consideration
• 1 professional reference required for consideration
• Experience with InSync EMR preferred

• Candidates who have previously worked for this facility as PRN or permanent staff must have been separated for at least 1 calendar year to be eligible as a traveler
