American Traveler is seeking a Physical Therapist with 2+ years of experience for a 13-week Home Health assignment, providing patient care in a community-based setting using Cerner EMR.

Details

• Home health care setting with field-based patient visits
• Patient population includes geriatric, joint replacement, stroke, CHF/COPD, and Parkinson's diagnoses
• Approximately 5 patients treated per day, varying by visit type or evaluation
• Day shift schedule, 8:00am–4:30pm
• Self-scheduling model
• No weekend or holiday requirements
• Works alongside PTAs who are supervised by the PT
• Charting system is Cerner, which is transitioning to Epic

Requirements

• Minimum 2 years of physical therapy experience required
• Home health experience required
• OASIS experience required
• Current BLS certification required
• Active PT license required by start date; license required if applicable
• 2 references required for consideration: 1 supervisor reference from within the last 12 months of relevant experience, and 1 peer or supervisor reference from the same specialty within the last 3 years

Additional Information

• Lymphedema certification is a plus
• Orientation includes computer-based training followed by 1-on-1 field shadowing (8 hours total)
• Solid-color scrubs in any color and closed-toe shoes required
• Former permanent staff must have been separated for at least 6 months and require facility approval before being considered
• Candidates residing within 80 miles of the facility are considered local and may be subject to a reduced pay rate
