American Traveler is seeking a Physical Therapist Assistant for a post-acute home health assignment requiring 2 years of experience and an active CA PT license.

Details

• Provide physical therapy services in a post-acute home health setting
• Shift is 8-hour days with every other weekend coverage
• Patient population includes individuals requiring rehabilitation and physical therapy services at home
• Use of HomeCare Home Base (HCHB) EMR is preferred
• Length of assignment is 13 weeks

Requirements

• Active Physical Therapist Assistant license in California required
• Current BLS certification required
• Current NPI number required
• Minimum 2 years of experience as a PTA in a healthcare setting required
• Home health experience, including OASIS, required
• Community or home health experience preferred
• Valid driver's license and proof of automobile insurance required
• Good oral and written communication skills and effective organizational skills required
• License required if applicable

Additional Information

• Provide therapy services according to a physician's plan of care, assess and update therapeutic status, and participate in plan of care development
• Evaluate home environment and instruct patients, families, and caregivers in the use of therapeutic appliances
• Supervise and direct physical therapy treatments and home health aides as needed
• Prepare and submit clinical and progress summaries, and communicate changes in patient condition to physicians
• Mileage is billable per GSA guidelines with supervisor-signed timesheets required
• Twice-monthly meetings to have hard copies of mileage timesheets signed; photocopies may be submitted via email
• Candidates may not have been employed by CommonSpirit, CHI, or Dignity facilities in the past year unless employed as PRN through an outside agency
• References from a supervisor within the last 12 months and a peer or supervisor within the last 3 years are required
• Scrubs or professional attire may be required depending on assignment norms
