American Traveler seeks a Physical Therapist with 2+ years of recent home health experience and an active CA PT license for a day shift home health assignment.

Details

• Home health setting serving the San Jose, South Bay, and Santa Clara County areas
• Day shift schedule with 5x8 hour shifts from 8:00 AM to 5:00 PM
• Patient care is delivered in the patient's home environment

Requirements

• Current and active CA PT license required
• Current BLS certification required
• Minimum 2 years of recent physical therapy experience with at least some in home health
• Must meet the 50-mile radius rule for eligibility
• Return staff must have been gone at least 1 year to be eligible
• License required if applicable

Additional Information

• Must be available to go for lab work within 3-5 days of signing ACA
• Only 1 holiday request for time off (RTO) will be approved
• Responsibilities include delivering PT services, documenting home health visits, and managing caseloads across designated geographic areas
• Submission consideration requires recent supervisor references and copies of all certifications
