American Traveler seeks an experienced Physical Therapist for a day shift Home Health position requiring BLS and 1+ year of Home Health experience.

Details

• Work in a skilled home health setting covering various patient populations including med/surg, cardiac, orthopedic, neuro, post-surgical, respiratory, and trauma cases
• Typical caseload of 6-8 home visits per day, self-scheduled based on office needs
• Patient population includes those with COPD, CHF, pneumonia, CVA, Parkinson's, trachs, and amputations (AKA, BKA)
• Uses BayCare documentation systems, including BayCare Forms, BayCare Scheduling, Ace Timesheet, and Cerner (preferred)
• Day shifts with hours from 8:00 AM to 5:00 PM, 5x8 hour shifts per week
• Requires 1-2 weekends per contract
• Travelers may float within the hospital or to affiliate hospitals

Requirements

• Current BLS certification required
• State PT license or compact license required at time of consideration (license required if applicable)
• Minimum 1 year of recent Home Health PT experience required
• OASIS training/experience preferred
• Valid driver’s license and reliable car required for home visits
• Must be COVID vaccinated or have approved exemption on file
• Annual flu vaccination or signed declination with mandatory masking
• Experience with Cerner charting system preferred

Additional Information

• Responsible for patient assessment, family education, home exercise program development, equipment and device use, and billing documentation
• Tasks include safety/injury prevention, ortho and neuro rehab, wheelchair assessment, and abuse recognition
• Must pass a facility EKG exam (up to 3 attempts allowed)
• Average of 4 hours spent on required onboarding modules, completed pre-start and annually
• Solid color scrubs permitted
• Contracted staff may not request RTO; any time off must be coordinated on-site with unit manager
• Travelers must live at least 100 miles from the facility to be eligible
• Smoke-free work environment
• Permanent employees must wait at least 1 year before returning as a traveler, with exceptions for certain specialties
• Return travelers eligible only for specific specialties not including PT
