American Traveler is seeking a Physical Therapist with home health experience, BLS certification, and a valid PT license for day shifts in a home care setting.

Details

• Home health setting covering Sarasota and Charlotte County
• Patients include med/surg, ortho, neuro, post-surgical, cardiac, respiratory, and geriatric cases
• Typical caseload of 6-8 skilled home care visits per day
• Patients served include those with CVA, COPD, pneumonia, CHF, falls, Parkinson's, trauma, trachs, and amputations
• Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet for documentation
• Cerner charting experience is preferred
• Day shift schedule: 5x8 hours, typically 8:00am to 5:00pm
• 1-2 weekends per contract may be required
• Visits and hours are self-scheduled based on the needs of the HomeCare office
• May require floating within the hospital or to affiliate hospitals

Requirements

• Active FL or compact Physical Therapist license required if applicable
• Current BLS certification required
• Minimum 1 year of home health physical therapy experience
• Valid driver’s license and personal vehicle required for patient visits
• Experience with OASIS preferred
• Ability to use equipment such as gait belts, walkers, sit-to-stand lifts, sliding boards, wheelchairs, and supplemental oxygen
• Must have documentation and billing skills
• Must be COVID vaccinated or have an exemption form on file
• Flu shot required or declination with mandatory mask usage
• Must live at least 100 miles from the facility

Additional Information

• Duties include patient assessment, developing and implementing home exercise programs, equipment management, medication review, and patient/family education
• Responsibilities include recognizing signs and symptoms of physical, sexual, and psychological abuse and ensuring safety and injury prevention
• Laptop, MiFi, and portable printer provided for documentation and scheduling
• Permitted to wear any solid color scrubs
• Passing a facility EKG exam is required prior to start (up to 3 attempts allowed)
• Facility is smoke-free
• Compliance documents are due 10 days before the start date
• Return staff must wait at least 1 year before returning as a traveler
• RTO is not accepted in the contract and must be discussed with the manager onsite if needed
• Pre-start and annual online modules are required, averaging 4 hours for allied health staff
