Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent skilled home health experience and OASIS proficiency.

Details

• Home health setting serving adult patients in the region
• Typical caseload of 4-5 adult patients per day, up to 6 visits daily
• 1:1 patient care model with productivity based on 27-30 points per week
• Shift is Monday through Friday, 8 hour days (8:00 am – 4:30 pm), with no weekends required
• No on-call requirements
• System groups clients to minimize driving; occasional maximum territory radius may be up to 100 miles
• Mileage is reimbursed at IRS standard rates
• Charting is done in Home Care Home Base; experience required
• Free parking provided
• Any color scrubs accepted

Requirements

• Active WA PT license required; license in-hand if applicable
• Minimum one year of recent skilled home health PT experience within the last three years
• OASIS experience required for Start of Care visits
• Current BCLS certification required
• Proficiency in Home Care Home Base (HCHB) strongly preferred
• Must have an active driver's license and personal transportation
• Completion of Hospice/Home Health Checklist required for consideration
• Timely communication and documentation skills are mandatory

• Primary responsibility is providing skilled therapy visits and completing documentation thoroughly and on time
• Travelers are expected to return documentation tools at end of assignment
• Territories and client lists are managed to support efficient scheduling
• First-time travelers are accepted for this assignment
• Travel pairs accepted, each assigned to separate territories and must have own transportation
• All time off requests must be submitted with application
• Former perm staff must be separated for at least 6 months before returning as a traveler and require HR approval
