Requirements

Current PT license required, if applicable
Minimum of 2 years of recent physical therapy experience
DOB and Social Security number required for consideration
COVID vaccination card required
RQI certifications are not accepted

Permanent address must be more than 60 miles from the facility
Candidates cannot have been employed as permanent staff at any LifePoint facility within the last 12 months
